Is it design contradiction or a hint? Maybe it's Decoy Octopus disguised as Big Boss. We already have Psycho Mantis and Ocelot in this game by the looks of it.
Ishmael may even be Decoy Octopus, who knows? That would explain why they both sound like Kiefer Sutherland.
