The Fiend Rose is a rare plant found during the endgame Helltide events in Diablo IV. These events become accessible once you reach World Tier 3: Nightmare difficulty, which is unlocked after completing the Campaign and Cathedral of Light Capstone Dungeon in Kyovashad at World Tier 2: Veteran. Helltide events are large-scale occurrences that occur across regions, spawning powerful demons infused with the essence of Lilith, and should not be attempted solo.
