The AI is smart either way. The title just made that up. The enemies seem to be more vigorous about killing you in hard from what the recent demo shows.

You do realize Metal Gear games of the past either came out on PC way later, got bad ports, or never came at all to PC right? I do hope it comes to PC, but it's really a small hope considering it's a Japanese dev.

There was health regen in MGS2 and 3. Once your LIFE got low enough, you could either crouch or go prone and it would recharge up to a certain point.
