Annoying game conventions that need to go away

Video gaming is not quite what it used to be, from the side-scrolling pew-pew of Contra to the mind-numbing intensity of games like Modern Warfare. Even so, as the years go by, there seems to be many different trends that plague medium.

Whether it be gameplay, plot, character, or even simply control-based issues, there are many annoying conventions that would best serve developers and gamers alike by dying quietly. In no particular order, here are a few:

How did this get approved?

Also from the blurb, RE's controls were fine until RE5 when it turned to action. For a horror game the tank controls work well enough.
