Jolt: "Drug taking in Fallout 3 is an example that, like the character creation, there are implications to all of your choices through which bad will almost always follow good and vice versa. Arguably the game's 'Karma' system favours those who walk a line of neutrality, and the game perhaps makes it a little too easy to adjust your standing through minor acts of good and evil, but if you avoid doing this and just let things play out the way they would normally, your experience will be richer for it."
