Yeah this game is fabulous. It's refreshing playing a title that doesn't hold your hand in terms of difficulty and actually has a mature story.

Thinking of getting this but I haven't played any of the others, would I be lost or not understand the story at all?

Not at all. The only real connections between this game and the previous Shin Megami Tensei games are a few continuity nods here and there. The story itself is completely independent and easy to grasp.
