Because it's a glitch and usually takes a really odd set of circumstances before it presents itself.

Anything can cause a glith even in the most polished games. Failing hardware, corrupted installs or saves, net code, lag, or crappy connection.

Last one is normal. Mass effects face and body animations our some of the worst. Entire game looks like a glitch. Anyone know if that batman one is really a glitch or not?
