If you knew anything about the Metal Gear Solid history, you should know that there is a character called Psycho Mantis who can manipulate people's minds.

Ground Zeroes (takes place before Metal Gear Solid) probably shows a young Psycho Mantis, and this trailer is probably a hallucination brought on by him.

Big Boss lost his limbs , Eye, and ear in Metal Gear 2 so it fits perfectly with the man in the trailer. And the flower petals that are floating around him are the exact same ones from MGS 3 when he fights The Boss in the flowers.

From fox two phantoms were born. A Phantom is something that is seen heard or sensed but has no physical reality. Thats why Snake would have a hook as a hand because its not in reality. It could be Solid Snake in a coma in his last dying days because of the FOX-DIE and hes dreaming all this. The doctor says hes not so long left At the end you see a dinosaur eat a chopper = METAL Gear REX. The sound effect at the end is the exact same sound effect that plays from phycho mantis. The voice at the end say V has come through which also might suggest they are trying to clone Solid and all those patients are the failures. What more clues do you need that suggest this is a mgs game.

perhaps his missing arm is a metaphor for how the boss broke effectively "taking" his arm. Look at the flower petals floating around him at the start of the trailer. I think this is a huge dream sequence in which Big Boss is still struggling to come to terms with the boss' death. Phantom could also be a link about the boss in peace walker about funcional compensation for one part of her brain taking over the part that was damaged. Could also be to do with the boss being a phantom or ghost. Or of cause Snake being kidnapped for the les enfants terribles project but it doesnt quite make sense if these people sound eastern european. Perhaps its also some metaphor for Snake thinking hes psychologically "sick" and sick for killing the woman he most deared and therefore trapped himself in a hospital dreamscape

At the same time look what it did. Before it was "oh its Metal Gear" anyone not a fan auto blew it off. You come back and pretend its a whole different game, those SAME people that blew off Ground Zeroes are all of a sudden excited and pumped up. Saw it early here too, some of the people were going "wow that looked awesome" then the MGS fans kicked in and it went back to "oh whatever. looks alright"

Now imagine the average person watching the VGA's that don't run all over the internet. Its going to take them a little while to figure it out.

Phantom Pain is usually related to so called phantom limbs, a phenomena in which you feel pain where an amputated limb used to be.

I'm thinking this video is like the operation in which they tried to clone Big Boss or something.

The phantom pain could refer to the loss the clones feel at being lesser versions of something, or which Big Boss feels at having lost a part of himself (his hand / forearm) for cloning.

Also there's the whole "from Fox two phantoms were born."

I think its Big Boss. At the end of Metal Gear 2 he lost his limbs his eye and his ear. Seems to fit perfectly with the man in the trailer. But its also weird that he lost his eye in Mgs 3 so im not sure why they decided to put that in.
