regardless i wont be seeing tali's face... just managed to get her killed ffs :/

well I was replaying a save for ME3 originally i'd only gotten mordin and legion killed (they were both loyal aswell :/) and i wanted them alive... for some reason this time using tali whilst she's loyal to do the same thing she survived last time (open the doors) got her killed this time ffs lol

Well replay the last mission again if possible...the survivors depend on the leaders you choose when the games asks you to

I chose:
Legion for tube thing
Samara for "protection" part
Miranda when i leave them to kick collector's ass
Mordin for escort(pretty high dying rate of this guy :P)

and all of them survived :D

Its not about loyality...its about who is fit for the job in that situation...

I chose:
Tali through the tube
Garrus to lead the second team (even after the final cut-scene b4 the final battle)
Jack to supply the biotic force-field
Mordin to lead the crew back to the ship
For the final battle I choose Samara and Thane

No one died even though Grunt did fall down/look unconscious.

I earn all their loyalty btw.

Loyalty sometimes can and can not matter.
For example my first play-through even with loyal.

Zaeed led the second team.
Jack supply the force-field.
Legion went through the tube.
Mordin lead the crew back the ship.
Use Garrus and Samara in the final battle.

Zaeed,Grunt and Legion(not in the tube but during the final battle) end up dying somehow.

The survivors depends on 4 things :
Did you upgrade the Normandy ?
Can the guys you pick fulfill their role ? (like if you send Jacob in the tube when he volunteers to do so, he's going to die, even if he's loyal; same when you put Zaeed as a team leader, he's a great fighter, but not a "people person", and last example, if you pick Miranda for the biotic barrier, she can't keep it up long enough and someone else dies)
Are they loyal ?
Who do you take with you for the last fight ? Cause when you fight the baby reaper, the rest of the team is supposed to stay behind to buy you time, so what can get some of your squadmates killed at that stage is if you take the best fighters with you (Zaeed, Grunt, and Garrus I think) instead of leaving them behind.

Hopefully we get Wrex back in our squad. I have my profile ready from Mass Effect 1 and 2, all characters still alive.........except for Kaido.

Just as an aside, if I have multiple save files, which one gets ported over?

Assuming ME3 will have a launcher like ME2, you can import all your saves from inside the launcher. Then select which save you want to run fist from within the game.
