Amazing how bad this game looks already. We keep acting like this gen doesn't feel like an update but I think we're starting to see it.
The game started really strong in that hospital, but after a couple of hours I wondered if it was even an mgs game I was playing.
You forgot the biggest clue of them all, the fact that the character's hair looks lighter than Snake's and Big Boss, hinting at Liquid, although Solidus also has light hair. That said, Liquid possibly dyed his hair that colour in MGS, I can't recall.
The age of the character would fit Liquid also, and the British flag in the hospital also some hint seeing as Liquid has ties to the UK.
"You've been in a coma for quite some time".
What's interesting is could this be the true story of what happened to Liquid Snake post Shadow Moses and could it be that Liquid is suffering phantom pain from who he is cloned from... Big Boss, hence some of the visions in the trailer.
Just to add. I thought this was actually a nightmare mini game for MGS V developed by a different studio.

**** NO! That's the dumbest thing I've ever heard. How are people taking the fact that Ocelot took dead Liquids arm and turning it into this? It would be more plausible for it to be Ocelot, but it isn't. It's so very obviously Big Boss, you'd have to be completely ignorant of the series to think anything else.

The answer is pretty obvious, no f'ing around with is that this and what if that, it's pretty simple:
The 'Phantom pain' is a chapter in MGS5 as in actually leading to a boss battle ie the specter you see floating around. The boss specialises in psychological tricks and the level is a product of it. Quite frankly I'm shaking my head at why this hasn't been suggested when a big deal of Metal Gear involves some sort of boss like that.
